INSTRUCTOR’S CONTRACT

Between:  The Edmonton Rug Hooking Guild (hereinafter referred to as the ERHG)

and –   __________________________________________________ (hereinafter referred to as the Instructor)

1.  The Instructor agrees to present the class “_______________________________________________________”

from __________________________ through __________________________.  Classroom sessions will be held from ________ a.m. to ________ p.m.  Lunch breaks will be one hour or a period agreed upon by the group.

2.  All monies referred to in this contract shall be paid in Canadian funds.

3.  ERHG shall pay the instructor $________ per student, per day, for a total of ___ days for up to a maximum of ____ students.This fee shall be paid on the last day of instruction, which is ­­­­­­­­­­­­­­­­­­__________________________.

4.  ERHG will reserve and pay for accommodations and meals for the instructor from ________________ evening through ________________, and including ________________.Receipts will be required for any meals purchased.

5.  ERHG shall pay air travel expenses incurred by the Instructor based on the average cost of an economy rate return air fare from the airport closest to the Instructor’s home to Edmonton, Alberta.Instructor is required to purchase flight cancellation insurance, limiting the liability to the cost of the flight, should the Instructor be unable to attend or the course is cancelled.ERHG will also pay for two suitcases, to a maximum of $_______.Any Instructor who wished to drive will be paid a mileage allowance not exceeding the above air fare.Appropriate receipts and mileage must be submitted to ERHG prior to being paid for instruction.

6.  Any decision regarding cancellation of a class due to low enrollment will be made by ERHG by the teacher liaison.The Instructor will be notified by telephone or email within three (3) days or seventy-two (72) hours of the decision.ERHG will follow up the telephone notification with a letter confirming the cancellation.

7.  The Instructor will notify ERHG by telephone or email within three (3) days or seventy-two (72) hours of determining any inability to fulfill this contract.The Instructor will follow up the telephone notification with a letter confirming their withdrawal.

8.  The Instructor agrees to send the following items to ERHG:

  • a signed copy of this contract;
  • a brief course outline, listing the goals for the class and the skills/content that will be taught;
  • a photograph of a completed rug(s) illustrating content of the class.Instructor is to suggest pre-requisite requirements or levels of experience and/or proficiency required of students (i.e. intermediated and an advanced hooking levels; should have attended a class in ___________________, or suitable for all levels); and
  • a short (one paragraph) biography of the instructor, with a photograph if desired.

9.  ERHG undertakes to provide a class list with contact information, to the Instructor by _____________________.Contact information for any additional students enrolled in the Instructor’s class after that date will be sent to the Instructor as they are received.

10. The Instructor undertakes to communicate with the students by ________________________________ and to provide adequate information to and contact with students regarding what they may need for the class.
